Community Champions
Purpose of the initiative
- To recognise exemplary service delivered by health professionals and others who regularly work with carers in the community
- To raise awareness of best practice
- To encourage others who may not be aware of the specific needs of family carers to adopt a collaborative approach
Who can be nominated?
Anyone and any organisation including family members, fellow carers and volunteers who have provided nominating carers or a member of their family with exceptional service or first class treatment or care.
Who can make a nomination?
Any unpaid care-giver living or working in Wolverhampton or caring for someone living and working in the City
How does it work?
- Nomination - all carers in Wolverhampton are free to nominate a community champion by completing this nomination form.
- Administration - the Omega Team will check that applications are complete. We will attempt to collect as much background information as possible to assist Forum Officers and Members with their deliberations.
- Validation - officers will consider the nomination and decide whether to refer the matter to the Forum or reject it without comment. Every Officer should have a right of veto.
- Approval - members of the Wolverhampton Carer Forum will carefully consider the nomination and have an opportunity to discuss it together before reaching a preliminary decision, based upon a unanimous vote. Their preliminary decision will be subject to consultation and such further enquiries as considered necessary. Members who have knowledge of the type of service under consideration will be encouraged to share their experiences to ensure that the perceived added value described by the nominee is not routinely available.
- Ratification - the final decision to award a certificate should be postponed until a period of at least two months has elapsed. This delay will conveniently allow time for a period of reflection and to check details in need of further clarification; it will also enable members to canvass the views of other carers in advance of the next scheduled Forum Meeting. A further, and final, vote will be taken and arrangements made to present the Award to the recipient.
- Award - the Chairman will sign a certificate and present it. Omega will make a public announcement on behalf of the Forum. The process can be stopped at any stage in the event that adverse or contradictory information is received. Any such information will be discussed by Officers, who will make a recommendation to the Forum.
